Referral Process

• Notify KODA of every death in your facility by calling the referral line at 800-525-3456.

• Confirm the suitability of a potential donor by providing:

•        Patient's name, age, and cause of death

•        Patient's diagnosis

•        Date of admission

•        Past medical history

•        Positive culture results

•        Last WBC

•        Provide the coordinator with next of kin information.

The coordinator will ask the questions above to determine donor suitability. If the patient is suitable for donation, the coordinator will contact the family about donation. If the patient wished to donate, and the family consents,  a medical/social interview is completed with the family.  We will then ask you to perform Eye Prep on the patient. The recovery technician will bring all materials needed with them to the hospital.
