Dr. Foulks graduated from Columbia College of Physicians & Surgeons with a residency at the Duke University Medical Center in the area of ophthalmic patient care.  He performed a two year fellowship at Harvard Medical School and the Massachusetts Eye & Ear Infirmary in cornea and external eye disease.  Dr. Foulks has served as faculty for 17 years at the Duke University Eye Center and 7 years at the University of Pittsburgh School of Medicine.  Dr. Foulks has been at the University of Louisville as a Professor of Ophthalmology since 2003.  He has also been the Director of Cornea Services and Co-Medical Director of the University of Louisville Lions Eye Bank since 2003.

Dr. John is a Board-Certified Ophthalmologist and Lasik specialist.  He is fellowship trained in Cornea/External diseases and Refractive surgery from Wilmer Institute at the John Hopkins Hospital in Baltimore, MD.  Dr. John has been an Associate Professor at the University of Louisville Department of Ophthalmology and Visual Sciences, where he received the Outstanding Faculty Award.  He is also currently the Co-Medical Director for the University of Louisville Lions Eye Bank

Retired Founding Executive Director

 

James Martin (Jim) received his education at the University of Kentucky, and the Kentucky School of Mortuary Science where he received his degree in mortuary science.  He is currently a licensed funeral director and embalmer in Kentucky and a Certified Eye Bank Technician by the Eye Bank Association of America (EBAA).  He has served on various committees in the EBAA, including the Finance, Meetings, International Relations, and the Constitution and By-Laws Committees.

 

In 1986, Jim was asked to restructure the state's first eye bank, the Kentucky Lions Eye Bank located in Louisville.  Under his direction an estimated 6,000 to 6,500 corneas have been transplanted.  He served as the first president of the University of Louisville Lions Club. He currently is a member of the Board and Membership Committee Chair.

Eye Bank Director

 

Joe has worked for the  University of Louisville Lions Eye Bank since 1993.  Prior to accepting his current position as Acting Executive Director, he was the Director of Quality Assurance with the Eye Bank and Senior Tissue Recovery Coordinator for the Kentucky Organ Donor Affiliates.

 

Joe is a licensed funeral director and embalmer in Indiana and Kentucky.  Additionally, he holds three national certifications.  He is a Certified Funeral Service Practitioner  (CFSP), Certified Tissue Bank Specialist (CTBS), and a Certified Eye Bank Technician (CEBT).

Quality Assurance Director

 

Susan is a Registered Nurse and a Certified Eye Bank Technician.  She is currently enrolled at the University of Louisville working toward her Bachelor's degree in Communication.  She has been with the University of Louisville Lions Eye Bank since 2003.  Additionally, Susan has experience working in bone marrow transplant, Hospice, and IV therapy/PICC team.

Aaron has worked in either tissue or eye banking since 1997. Prior to accepting his current position as the Eye Bank Technical Director, Aaron worked for Kentucky Organ Donor Affiliates as a Tissue Recovery Coordinator. He has held certifications as a Certified Eye Bank Technician (CEBT) and as a Certified Tissue Bank Specialist (CTBS). He also holds a current certification as an Emergency Medical Technician (EMT-B) and is working toward becoming a Paramedic.

Eye Bank Technician

 

Scot has worked with the University of Louisville Lions Eye Bank as a Recovery Technician since September 2004. Scot moved into a full-time position with the University of Louisville Lions Eye Bank in September 2006, leaving behind a 16 year career as a Medical Laboratory Technician.
