Dear healthcare professional,

We have designed this section with your needs in mind.  We hope it is helpful and easy to use.  We appreciate everything you do in assisting us with the donation process.  We cannot do it without you.  You are a vital piece to donation.  It takes a team . . . and together we can make a difference.

The University of Louisville Lions Eye Bank is available 24 hours a day, 7 days a week to provide recovery, education and information services on all aspects of the eye donation process.  Our hospital development department can provide you with education services and information for both medical professionals and the general public.  Please call the Hospital Development Department at (502)852-5457 for more information or complete and fax the Needs Assessment Form below back to us.

We also ask that after a donor from your facility, that you let us know your feelings about our recovery by completing and faxing the Eye Donor Survey below back to us.  Your input is extremely helpful to us, so that we can constantly improve our service to your facility as well as the donor family.
